People who are around the age of 60 or senior citizens, some of them may not necessarily have birth certificate and/or matriculation certificate as a supporting document for date of birth for the passport. In those days it is possibly for someone to have not registered the birth and hence would have not received birth certificate and even those people who had it, there is very likelihood that it is either misplaced or lost. So to help facilitate those people apply for passport, Annexure A for passport is an alternative. This affidavit is declaration as to the deponent is literate or an illiterate. The deponent also declares that content have been read and they clearly understand the declaration in the affidavit. Annexure A for passport is executed on a minimum value on an e stamp paper and the same document needs to be notarized.
